REDUCING BAGS AND SAGS: EYE SURGERY The procedure to correct fullness and bagginess around the eyes is called a blepharoplasty. The result is long lasting and may never have to be repeated. As we age, the skin and tissues around the eyes start to loosen and relax. In the upper lids, extra tissue develops which creates a fold. This fold can actually fall down onto the eyelashes. Occasionally, if there is a great deal of extra skin in the upper lids, vision can be impaired. In the lower lids, there may be puffiness as well as lines and wrinkles from loose skin. Heredity may play a big role in the timing and degree of change that occurs. Some young men and women may have fullness in the upper lids and puffiness in the lower lids at an early age, and consideration of eyelid surgery at younger ages may be appropriate as well. |

There is a natural cushion of fatty material around the eyeball. This is held in position by a thin membrane. It is the relaxation of this membrane that causes the puffiness in the lower lids as well as some of the puffiness in the upper lids. Eyelid surgery removes the extra skin and tissue in the upper lids, eliminates the puffiness in the lower lids and tightens some of the skin as well. As a result, the eyes look more awake and the face has a more rested and vital look. THE SURGERY: The operation can be done on an outpatient basis at one of the ambulatory surgery centers or, on occasion, can be done in the office. The anesthetic is usually a combination of local anesthetic with intravenous sedation. This causes a twilight sleep which most patients find quite agreeable. In the upper eyelid, an incision is made in the natural crease line, and the excess tissue is removed. The incision is then closed with a stitch that runs underneath the skin. This stitch is usually removed about one week later. In the lower lid, an incision is sometimes made on the inside lower part of the eyelid. Through this small incision, the fat causing the puffiness can be removed. In this case, there is no external incision line. If the skin itself needs tightening, a small incision is made on the outside of the lower eyelid just below the eyelashes, and the extra skin is removed. Sometimes, a light acid peel is done in the lower eyelids in conjunction with the blepharoplasty to lighten dark circles and smooth the skin.
